The Leadership Delta Application requires the following information from Applicants:
- General Information
- Name, contact information, and emergency contact information.
- Academic Information
- College/University name and contact info, major, date of graduation, classification, and GPA.
- Delta Sigma Theta Information
- Chapter name, membership number, and advisor name and contact information.
- Leadership Information
- Pre-collegiate and collegiate leadership positions.
- Community Involvement
- Pre-collegiate and collegiate community involvement activities.
- Honors & Awards Received
- Pre-collegiate and collegiate honors and awards received.
- Statement of Purpose
- Statement of purpose detailing academic, leadership, and professional interests and how participation in the Leadership Delta program can help. Not to exceed 500 words.
- Two (2) 'Passport-size' Photographs
- Resume
- Two (2) Letters of Recommendation (One from School Official and the other from an Active Delta)
- A Current Transcript
